INTRODUCTION

The Kentucky Department for Public Health (KDPH), Healthy Communities Grant Program is seeking applications from local health departments and partners to develop a 5-2-1-0 Healthy Numbers for Kentucky Families campaign. The purpose of the campaign is to increase the awareness of evidenced-based health behaviors and promote best practices around healthy eating and physical activity in a variety of community settings.

 

The 5-2-1-0 Healthy Numbers for Kentucky Families campaign is a message promoting evidence-based behaviors that prevent and reduce obesity.

 

5-2-1-0 Healthy Numbers for Kentucky Families

 

Five (5): Eat five or more servings of fruits and vegetables each day.

                Two (2): Limit screen time to no more than two hours a day.

                One (1): Be physically active at least one hour a day.

                Zero (0): Zero Sugary Drinks.

The campaign is designed to give parents, community partners, healthcare professionals, employers, schools and child care providers a consistent message that is easy to remember. The 5-2-1-0 Healthy Numbers for Kentucky Families campaign is not a program. The 5-2-1-0 graphics and tools have been designed as part of a messaging framework with the goal of increasing of the evidence based health behaviors. Behaviors are influenced by many factors including family, friends, local surroundings, built environment and community. In order to bring about behavior c

hange, communities need environments where healthy eating and physical activity are easily accessible and supported by policies.

Applicants will develop a plan with partners to promote the 5-2-1-0 campaign in a priority setting and targeted audience. Applicants may choose to focus on all or one of the 5-2-1-0 behaviors.

Funding is available for six $1000 mini grants.

Criteria:

  • Kentucky local health departments must be the fiscal agent.
  • Participate in a 5-2-1-0 webinar. (Date to be announced.)
  • Convene a minimum of three community partners to develop plan for a 5-2-1-0 community wide campaign. Choose at least one setting and one priority audience to target. Applicants may choose to promote the 5-2-1-0 campaign then focus on one of the 5-2-1-0 behaviors.
  • Participate in at least one technical assistance networking call.
  • Provide letters of collaboration from at least three partners.
  • Submit a written application, of 500 words or less.
  • Spend funding by June 30, 2016.
  • Funding cannot be used for construction or food for meeting purposes. A maximum of 50% of grant funds may be used for staff time.
  • Utilize a tracking tool provided by the KDPH Healthy Communities Program to track activities including the event type, audience size, materials provided, topic covered, and technical assistance provided. Submit tracking tool as final report by August 15, 2016.

Application:

  • (30 points) List at least three partners and provide letters of collaboration
  • (50 points) Submit a written description (500 words or less) on how you will work with partners to develop a plan to promote the 5-2-1-0 Healthy Numbers for Kentucky Families Campaign and describe how setting(s), priority audience and behavior(s) will be identified

Grant funding can be used for health department staff time and indirect costs. Remaining funding can be used for incentives for each of the target audience to participate in the trainings provided by the health department.

Examples of Best Practices for 5-2-1-0

Five (5): Increase Fruit and Vegetables

  • Improve opportunities to purchase fruit and vegetables by increasing healthy retail or farmers markets
  • Promote the availability of healthier foods and nutrition in early care and education, schools and worksites
  • Encourage food system support

Two (2): Decrease Screen Time

  • Reduce screen time in public service venues for children in schools, day care centers, and afterschool programs

One (1): Increase Physical Activity

•     Create or enhance opportunities for physical activity

•     Enhance physical education and activity in schools and physical activity in child care settings

•     Improve street-scale and community-scale design

Zero (0): Decrease Sugary Drinks

  • Ensure ready access to potable drinking water
  • Limit access to Sugar Sweetened Beverages (SSB)
  • Promote access to and consumption of more healthful alternatives to SSB
  • Limit marketing of SSB and minimize marketing’s impact on children
  • Decrease the relative cost of more healthful beverage alternatives through differential pricing of SSB
  • Include screening and counseling about SSB consumption as part of routine medical care
  • Expand the knowledge and skills of medical care providers to conduct nutrition screening and counseling regarding SSB consumption
